How To Enrol

We have simplified the enrolment process to make it hassle-free for you. We require patients to complete the registration/patient form before your first appointment can be scheduled.

NZ Citizens
Birth Certificate or Passport
If only a Birth Certificate or Certificate of Citizenship is available, we will require another form of photo 1.0 (e.g. Driver’s License), to confirm your identity.
NON-NZ Citizens
Passport with VISA
Note:
If you have less than 2 years of Visas you will NOT qualify for Government subsidised health care. We will therefore have to charge you as a visitor to New Zealand.
Please ensure that all of your details are correct, including medications. This would assist us in completing your enrolment in a timely manner.
